HOW WE CAN SUPPORT YOU

The National Disability Insurance Scheme (NDIS) is a government initiative in Australia designed to provide support and services to people with a permanent and significant disability. It offers funding and resources to assist individuals in achieving their goals and improving their quality of life.

To be eligible for the NDIS, you must be an Australian citizen or resident with a permanent and significant disability, and you must be under 65 years old when you first apply.

To apply, you need to fill out an Access Request Form and provide evidence of your disability. This should include reports and assessments from health professionals. The forms can be obtained from the NDIS website or by contacting an NDIS office.

The NDIS provides a range of support services such as personal care, community access, therapy, domestic help, and essential aids or equipment. These services are customized to meet your individual needs and goals based on your functional capacity.

The NDIS does not cover support if it is: - Not related to a person's disability. - A duplicate of another support within a participants personal support plan. - A normal daily living expense (e.g. rent, groceries, ticket to the movies). - Better addressed through an alternative support system (e.g. healthcare system). - Likely to cause harm to a participant or pose a risk to others. - Related to income replacement.

NDIS funding can be managed one of three ways: NDIA-managed, plan-managed, or self-managed. Each option offers different levels of control over how the funds are used.

Yes, you can choose your own service providers with the NDIS. This ensures that the services you receive meet your specific needs and preferences, supporting your right to choice and control over your life.

An NDIS plan outlines your goals and the supports funded by the NDIS. Developed with you, it is reviewed periodically to ensure it meets your needs. The plan includes three key areas: Core (daily activities), Capital (investments like assistive technologies), and Capacity Building (to help you build independence and skills).

The NDIS understands that while permanent disability is permanent, how it impacts you can change during your life. As such, if your circumstances change or your plan is not meeting your needs, you can request a plan review. This can be done by contacting your NDIS planner or the National Disability Insurance Agency (NDIA). If you choose to do this, you should get help from a support coordinator, disability advocacy group, occupational therapist or other health professional and peer support groups to give you the best chances at review.

The NDIS works alongside other government services but is designed to cover supports that are not provided by other areas like education, health, and transport.

A Support Coordinator helps you maximize your NDIS plan. They assist you in understanding and using your plan, connect you with NDIS providers and other services, and help you build the confidence and skills to manage your supports.

Plan management is when a provider supports you to manage funding in your NDIS plan. These providers are known as plan managers. Depending on your level of funding for plan management, they can help you:

• increase your financial and plan management skills

• learn how to self-manage your plan

• pay providers

• increase your choice of providers

• get NDIS plan budget reports and greater budget oversight.

When a provider is NDIS registered, they agree to follow special rules and procedures as outlined by the NDIS. It also means that you can use them as a service if your NDIS funding is NDIA managed.
