Client intake

For allied health professionals and support coordinators, please download and complete the form that is available below and return it to enquiries@cleanandpersonal.com.au, or complete the website submission.
As a coordinator, we will work with you in the interests of our mutual participant/s.

​​Once we receive the form, we pass it on to one of our great people in client relations.

 

They will give the client a call to become familiar with them and listen to their wants and needs, as well as noting any other information you have passed on in the referral form.

We will then place the job with the participant.

​

We will then follow up with you to let you know whether or not the service has been booked.

​

From then on and during the process, you can reach out to us via phone or  

E-Mail at any time. Such as for requests for information, to share information about the participant, to share any feedback, and more.

 

We pride ourselves on our responsiveness and endeavour to get back to you within one business day.
